The OPPORTUNITY

Barwon Health is seeking responses to this Request for Tender (RFT) BH_2015-1001 for the supply of at home community support services. The Barwon Health programs that will engage in the brokerage of these services are the Post Acute Care services, Transition Care Programs and Carer Respite Services.

Post Acute Care is funded by state governments and provides short term support, facilitating safe and timely discharge home for people who no longer need acute care & are able to safely return home with an appropriate package of community-based supports. The funding enables a rapid and flexible response to consumers’ needs to facilitate safe and timely discharge from the public hospital setting. The purchase and coordinate of short term community based services is in response to individually assessed needs.

The Transition Care Program is a jointly funded initiative of the Commonwealth Department of Social Services and the Victorian Department of Health. Transition Care – home based is provided in the community at the conclusion of an inpatient hospital episode to assist consumers in completing their restorative process and to optimise their functional capacity. Transition Care is goal orientated, time limited and therapy focussed. It provides older Australians with a ‘package’ of home based services including personal care, domestic help, community & social access, low intensity therapies, nursing support, meals and the provision of equipment. In many cases, it also provides active management for older persons and their families at the interface of the acute/subacute and aged care sectors, and enables time to make decisions and arrangements regarding their long term care needs.

Barwon Health Carer Respite Services is funded by Commonwealth & State governments.  The service is committed to supporting and maintaining the quality of life of carers and those for whom they care.  The service covers the Barwon South West region, and operates from 2 offices in Newcomb and Warrnambool.  Local government areas covered by Barwon Health Carer Respite Services include: City of Greater Geelong, Surf Coast Shire, Borough of Queenscliffe, Colac Otway Shire, City of Warrnambool, Glenelg, Moyne, Corangamite and Southern Grampians.

Barwon Health Carer Respite Services strives to provide high quality, flexible and innovative short term and emergency support options that are responsive to the needs of carers within the community.  We also provide a residential respite booking service, referrals to other relevant services and information about local services and providers to anyone in the general community.

Barwon Health Carer Respite Services supports family carers to take a break from their ongoing caring role by providing information, referral and care co-ordination as well as flexible, short term respite for the person they care for.  The Centre subcontracts some of these services out to external providers for the provision of social support, attendant care, help to attend appointments, shopping assistance, safety checks, meal preparation, domestic tasks, overnight respite and general gardening.
