Contract - C8092

(i) To raise funds for, and otherwise assist and support, individuals between the ages of 25 and 65 years old who are in the cycle of chronic homelessness; (ii) for the Outcomes to further an improvement of the personal, social and economic circumstances of a group of individuals experiencing chronic homelessness in communities in Victoria compared to a similar group of individuals who do not receive the Services; and (iii) to provide the Services as an appropriate means for the Proponent to take a relationship-based, trauma informed and strengths-based approach in the context of long-term assertive case management, with the aim to end the cycle of chronic homelessness for individuals participating in the Project.
