Monash Health is seeking responses to this ITS with the intention of appointing one or more Suppliers for the provision of TEN-19-0000031 Pharmacy Services to Monash Health Residential Services

Scope of the work:

Focus is on supply of medicines to residential facilities, including delivery and packing into dose administration aids. Other services include:

• Removal of unused or unwanted medicines
• New medicines brought to the residential facilities to be checked
• Provision of product information and consumer medicines information
• Clinical pharmacy services (including chart review) and education
• Residential Medication Management Review (RMMR)
• Quality Use of Medicines (QUM) Programs
