EH18-0206E for the provision of Eastern Health – Residential based - Transition Care Program.

Eastern Health is seeking responses from external residential care TCP facilities within the defined catchment area detailed in Annexure C.  Eastern Health currently outsources 45 bed based packages with approximately 1,372 bed days per month.  With a view to obtaining best coverage the ability for growth and staffing economies of scale, Eastern Health would consider awarding to a panel of providers the ability to meet a minimum of 15 and a maximum of 30 bed based packages per day on an ongoing basis. This would equate to between 465 and 1,395 bed days per month respectively.

Suppliers should be aware that this EOI process may lead to a tender process or directly to a negotiation with one or several preferred suppliers.

With a view to obtaining the best value for money, Eastern Health will consider separating the award to appoint a panel of providers to deliver the Services.

Specified care and services for transition care services must be provided in accordance Transition care programme Guidelines 2015. The guideline which can be located through the following link, indicates the minimum level of care that a Contractor must be able to provide:
