Monash Health are seeking suitable suppliers to be on a panel of preferred providers of brokered services in Occupational Therapy and Physiotherapy to patients.

The scope of the project includes providing services to the following Monash Health departments and community-based programs:
 Post-Acute Care (PAC)
 Sub Post-Acute Care (SPAC)
 Community Support Options (CSO)
 Hospital in the Home (HITH)
 Residential InReach (RESI)
 Rehabilitation in the Home (RITH)
 Disability Services
 Complex Care
 Residential Services
 Transition Care Program (TCP)
 In-Patient Services (including Acute, Sub Acute, Emergency and Mental Health), in situations where additional resources are required
 Other community-based programs (including those funded under Home and Community Care and the Commonwealth Home Support Program)
