Expressions of interest (EOI) are sought from providers of residential services to establish an arrangement with Eastern Health for the provision of transition care services in a bed based setting.

The Transition Care Program is a new Commonwealth and State Government funded program. The program targets older people at the conclusion of a hospital admission and aims to minimise the number of older people experiencing inappropriate extended hospital lengths of stay and being prematurely admitted to residential aged care.

Interested parties are invited to submit an EOI addressing the criteria outlined in the EOI documentation. The EOI documentation will be lodged on this site w/c Monday 19 December 2005.  Registrants prior to this date will be automatically alerted to the availability of the document.

Late, facsimiled or electronic submissions will not be accepted.
