Service Provider to provide the following services for the Embedded Youth Outreach Program:

• Joint after-hours outreach between local police members and a youth support service;
• Assessment and triage of young people (10-24) and their families who come into contact with police;
• Targeted referrals for young people; and
• Provide community engagement and practical support.
